School Year Registration Information
Applications will be made available at Kenter Canyon School beginning Tuesday, May 27, 2008 for the 2008—2009 school year for any new students in kindergarten through grade 5 who live within the LAUSD established boundaries for our school. To help us plan the appropriate number of classrooms, we are requesting that applications be returned no later than June 9, 2008. Applications will be marked with the time and date in which they arrive back to the office “completed” and will be processed in order of receipt.
Processing for completed “Kindergarten” applications will begin on June 13, 2008. Kindergarten assignments will be made no later than June 29, 2008 to enable families to enroll in the STAR program if they choose to do so. Grades 1-5 applications will be processed beginning August 28, 2008 and families will be notified via US Mail of room and teacher assignments. The assignments will also be posted on the first day of school, Wednesday, September 3, 2008 for all grades.
Kindergarten parents/guardians may request morning (8:00-11:20) or afternoon (11:30-2:50) classes; however, there is no guarantee that your request will be granted. We try our best to honor your request, but we also have an obligation to place students in accordance with LAUSD policies and our Charter agreement. Parents are only permitted to request an “a.m.” or “p.m.” space. As noted in the Kenter Parent Handbook, parents may not request specific teachers for any grade level.
For parents who wish to enroll their kindergarten age children in the before or after school STAR program, please contact the STAR Office for information and procedures. STAR Registration will be on Friday, July 18, 2008 beginning at 8:00 a.m.
Applications are not complete without the items listed below attached to the LAUSD Enrollment Forms. Partial applications cannot be accepted.
Requirements for Kindergarten:
• Proof of age (birth certificate, baptismal certificate, passport, health/vital statistics, affidavit) • Immunization records (mumps, measles, polio, etc.)
• PPD-mantoux skin test: Must be dated after September 3, 2007 if the child previously attended another school (if the mantoux skin test was done prior to September 3, 2007, it will need to be repeated.) • Proof of residence (utility bill i.e., gas, light or water but not telephone; rental agreement or contract lease agreement on embossed or stationery with logo) • Physical examination (must be done after September 3, 2007; if examination was prior to September 3, 2007, it will need to be repeated.) • Copy of IEP, if applicable.
Requirements for grades 1-5:
• Proof of age (birth certificate, baptismal certificate, passport, health/vital statistics, affidavit);
• Immunization records (mumps, measles, polio etc.)
• PPD-mantoux skin test results
• Proof of residence (utility bill i.e., gas, light or water but not phone; rental agreement or contract lease agreement)
• PAR (Pupil Accounting Report) from previous LAUSD School or Report card if attended a school other than LAUSD
